2010 Summer Camp Registration

There are a lot of forms to fill out! Packets of these forms will be available at the church office and in the youth room starting January 27th.

Camp Cost = $225 per student

If you would like to make payments, here is a suggested payment schedule:
$50 Deposit - February 17
$50 - by April 8
$50 - by May 12
$75 - by June 2

Mark your calendars now for a mandatory camp meeting on June 20th at 5 pm. Campers and parents must attend.

FBC Ocala Medical Release Form - Only needed if not already on file at the church. Note: this form must be notarized.  Notary Public services are available at the church office.
